We collect information directly when you:
Enroll in a program or purchase a service from NSD
Make a reimbursement request
Participate in optional surveys
Correspond with us (message boards, chat, email, portal messages, phone, or letters)
We also collect from third parties:
Business partners from whom you purchase a benefit or service provided by NSD
Partners who enroll you in NSD programs or services
Business partners and insurers on whose behalf NSD processes service requests
We do not knowingly collect Personal Information from anyone under 16 years of age.
